Long Term Care Services

St. Clair O’Connor Community Inc. has an accredited long term care facility funded by the Ministry of Health and Long Term Care. Our professional, dedicated, and highly trained staff provide 24-hour around the clock care to our residents. This facility is a home-like environment complete with its own dining area, big screen television in the common area, and individual rooms. There are always opportunities for those in long-term care to have as much access as possible to the community and other residents. Long-term care residents also participate regularly in SCOC programming.


Medical

Medical care for long-term residents is easily available and accessible. Weekly doctor visits can be scheduled with our on-call 24 hours a day licensed medical doctor.

A portable X-ray scanner and ultrasound service is available as ordered by the doctor. Also weekly laboratory services can be made available when needed.

Every Monday, Wednesday, and Thursday an on-site physiotherapist is available.

Prescription medication can be re-ordered through SCOC and can be delivered.

Nursing Care

Trained and professional nurses are available to residents when needed. We can provide services of personal support workers, registered practical nurses, and registered nurses.

Foot care nurses can also be made available when requested.


Continuous Quality Interim (CQI) Summary Report

The home has a formal interdisciplinary committee accountable for providing oversight and strategic direction on quality improvement activities; this will continue. The CQI Committee develops and approve the Terms of Reference to reflect its mandate. The home will ensure its CQI Committee fulfills the composition requirements and responsibilities described in the Fixing Long-Term Care Act, 2021. Read the full report.
